    Representations of Autism in Popular Media: Rhetorical Disability in The Good Doctor

    The Good Doctor is a medical drama about the workplace experiences of autistic savant Dr. Shaun Murphy. This program has been generally well-received by autism researchers because of its prosocial representation of an autistic person overcoming their disability. That said, many autistic people consider the program to be socially detrimental for essentially the same reason. To understand this disconnection, I interrogate disability in The Good Doctor via two lenses: (1) Foucauldian discourse analysis and (2) autoethnographic reflection. Comparing the results, I conclude that The Good Doctor, while well-intentioned, is rhetorically ambiguous because of its decision to depict a carefully controlled slice of autistic experiences. Thus, while this program may be enabling for some autistic people, the opposite may be true for others – a powerful lesson in the variety of autism manifestations

    A randomised controlled trial of pelvic floor muscle training for stages I and II pelvic organ prolapse

    Abstract Forty-seven women participated in a pilot study for a multi-centre randomized controlled trial of the effectiveness of pelvic floor muscle training (PFMT) for women with prolapse. Women with symptomatic stage I or II prolapse [measured by Pelvic Organ Prolapse Quantification (POP-Q)] were randomized to a 16-week physiotherapy intervention (PFMT and lifestyle advice; n=23) or a control group receiving a lifestyle advice sheet (n=24). Symptom severity and quality of life were measured via postal questionnaires. Blinded POP-Q was performed at baseline and follow-up. Intervention women had significantly greater improvement than controls in prolapse symptoms (mean score decrease 3.5 versus 0.1, p=0.021), were significantly more likely to have an improved prolapse stage (45% versus 0%, p=0.038) and were significantly more likely to say their prolapse was better (63% versus 24%, p=0.012). The data support the feasibility of a substantive trial of PFMT for prolapse. A multi-centre trial is underway.This study was funded by the Chief Scientist Office, Scottish Government (CZH/4/95)

    AbstractThe αβ interferons (IFNs) transiently induce genes through an IFN-stimulable DNA response element (ISRE). IFN-cell surface receptor interaction triggers the cytoplasmic activation of the complex primary transcription factor E, which on translocation and interaction with the ISRE initiates transcription. Whether E is activated directly through the receptor(s) or through a more classical second message pathway(s) and the roles of additional factors in the αβ and γ IFN responses remain to be established. Meanwhile analysis of mutants has revealed complexity and overlap in the α,β and γ IFN response pathways and the products of at least two viruses have been shown to inhibit IFN-inducible gene expression

    NaGdF4:Eu3+ Nanoparticles for Enhanced X-ray Excited Optical Imaging.

    X-ray luminescent nanoparticles (NPs), including lanthanide fluorides, have been evaluated for application to deep tissue in vivo molecular imaging using optical tomography. A combination of high material density, higher atomic number and efficient NIR luminescence from compatible lanthanide dopant ions indicates that particles that consist of ALnF4 (A = alkaline, Ln = lanthanide element) may offer a very attractive class of materials for high resolution, deep tissue imaging with X-ray excitation. NaGdF4:Eu3+ NPs produced an X-ray excited luminescence that was among the most efficient of nanomaterials that have been studied thus far. We have systematically studied factors such as (a) the crystal structure that changes the lattice environment of the doped Eu3+ ions within the unit cell; and extrinsic factors such as (b) a gold coating (with attendant biocompatibility) that couples to a plasmonic excitation, and (c) changes in the NPs surface properties via changes in the pH of the suspending medium-all with a significant impact on the X-ray excited luminescence of NaGdF4:Eu3+NPs. The luminescence from an optimally doped hexagonal phase NaGdF4:Eu3+ nanoparticle was 25% more intense compared to that of a cubic structure. We observed evidence of plasmonic reabsorption of midwavelength emission by a gold coating on hexagonal NaGdF4:Eu3+ NPs; fortunately, the NaGdF4:Eu3+ @Au core-shell NPs retained the efficient 5D0→7F4 NIR (692 nm) luminescence. The NaGdF4:Eu3+ NPs exhibited sensitivity to the ambient pH when excited by X-rays, an effect not seen with UV excitation. The sensitivity to the local environment can be understood in terms of the sensitivity of the excitons that are generated by the high energy X-rays (and not by UV photons) to crystal structure and to the surface state of the particles
